Constituted 2 August 1943 in the Organized Reserves as the Band, 95th Division

Organized 12 August 1943 while in active military service at the Louisiana Maneuver Area

Redesignated 1 December 1943 as the 95th Infantry Division Band

Inactivated 6 November 1945 at Camp Shelby, Mississippi

(Organized Reserves redesignated 25 March 1948 as the Organized Reserve Corps; redesignated 9 July 1952 as the Army Reserve)

Activated 16 May 1949 at Oklahoma City, Oklahoma

Redesignated 2 March 1959 as the 95th Division Band

Reorganized and redesignated 30 December 1967 as the band element of the Support Company and Band, 95th Division

Reorganized and redesignated 1 October 1970 as Headquarters Company and Band, 95th Support Battalion, an element of the 95th Division

Reorganized and redesignated 1 September 1971 as Headquarters Detachment and Band, 95th Support Battalion

Inactivated 15 October 1978 at Oklahoma City, Oklahoma

Band element withdrawn 16 November 1982 from Headquarters Detachment and Band, 95th Support Battalion, and relieved from assignment to the 95th Division; concurrently redesignated as the 895th Army Band and activated at Oklahoma City, Oklahoma

Reorganized and redesignated 17 September 1983 as the band element of Headquarters Company, 95th Division

Band element withdrawn 16 September 1988 from Headquarters Company, 95th Division; concurrently reorganized and redesignated as the 95th Division Band

(95th Division reorganized and redesignated 1 October 1994 as 95th Division [Institutional Training])

Reorganized and redesignated 16 October 2008 as the 395th Army Band; concurrently relieved from assignment to the 95th Division (Institutional Training)
