- Redesignated 16 October 1987 as Headquarters and Headquarters Company, 4th Battalion, 101st Aviation, and activated at Fort Campbell, Kentucky, as an element of the 101st Airborne Division (organic elements concurrently constituted and activated)
- Battalion relieved 16 September 2004 from assignment to the 101st Airborne Division and assignated to the 159th Combat Aviation Brigade, 101st Airborne Division
- Redesignated 1 October 2005 as the 4th Battalion, 101st Aviation Regiment
