- Organized and Federally recognized 1 June 1959 in the Pennsylvania Army National Guard at New Cumberland as the 28th Aviation Company and assigned to the 28th Infantry Division
- Reorganized and redesignated 1 April 1963 as Headquarters and Headquarters Company, 28th Aviation Battalion, and remained assigned to the 28th Infantry Division (concurrently, organic elements organized from existing units)
- (Company A allotted 1 February 1968 to the Virginia Army National Guard; Company A reallotted 1 April 1975 to the Pennsylvania Army National Guard and location of Headquarters changed to Harrisburg; Companies C and D allotted 1 September 1978 to the Virginia and South Carolina Army National Guard, respectively; Company C withdrawn 1 July 1986 from the Virginia Army National Guard)
- Headquarters and Headquarters Company, 28th Aviation Battalion, reorganized and redesignated 1 October 1986 as Headquarters and Headquarters Company, Aviation Brigade, 28th Infantry Division (remainder of 28th Aviation Battalion – hereafter separate lineages)
- Location changed 15 October 1991 to Annville
- Reorganized and redesignated 1 September 2007 as Headquarters and Headquarters Company, Combat Aviation Brigade, 28th Infantry Division
- Ordered into active Federal service 23 January 2009 at Annville; released from active Federal service 1 March 2010 and reverted to state control
