Organized in April 1917 as the American Red Cross hospital unit at Good Samaritan Hospital, Los Angeles, California

Reorganized and redesignated 14 March 1918 as Base Hospital No. 35 and mobilized at Los Angeles, California

Demobilized 6 May 1919 at Camp Kearney, California

Reconstituted 8 October 1936 in the Organized Reserves and consolidated with the 35th General Hospital (constituted in July 1923 in the Organized Reserves as General Hospital No. 35; redesignated 23 March 1925 as the 35th General Hospital; organized by 14 February 1929 at Los Angeles, California) and consolidated unit designated as the 35th General Hospital

Ordered into active military service 21 March 1943 and reorganized at New Orleans, Louisiana

Inactivated 10 December 1945 in the Philippine Islands

Redesignated 23 May 1947 as the 328th General Hospital

Activated 25 June 1947 at Salt Lake City, Utah

(Organized Reserves redesignated 25 March 1948 as the Organized Reserve Corps; redesignated 9 July 1952 as the Army Reserve)

Ordered into active military service 6 December 1990 at Salt Lake City, Utah; released from active military service 6 April 1991 and reverted to reserve status
