- Headquarters, Headquarters and Service Company as Headquarters and Headquarters Detachment, 65th Medical Group
- (Companies A, B, C, E, F, and G as the 431st, 432d, 433d, 434th, 435th, and 436th Medical Collecting Companies, respectively; Companies D and H as the 647th and 648th Medical Clearing Companies, respectively – hereafter separate lineages)
- Inactivated 31 January 1946 in Germany
- Activated 25 June 1958 in Korea
- Inactivated 21 June 1971 at Fort Lewis, Washington
- Redesignated 12 September 2007 as Headquarters and Headquarters Company, 65th Medical Brigade
- Activated 16 October 2008 in Korea
