Organized and Federally recognized 27 January 1947 in the West Virginia Army National Guard at Point Pleasant as the 3664th Ordnance Medium Maintenance Company

Reorganized and redesignated 15 September 1952 as the 3664th Ordnance Company

Ordered into active Federal service 25 September 1961 at Point Pleasant; released from active Federal service 16 August 1962 and reverted to state control

Reorganized and redesignated 1 March 1968 as the 3664th Light Maintenance Company

Reorganized and redesignated 1 February 1972 as the 3664th Maintenance Company

Consolidated 1 September 2005 with Detachment 1, 3664th Maintenance Company (organized and Federally recognized 24 January 2000 at Eleanor); consolidated unit designated as the 3664th Maintenance Company with Headquarters at Eleanor

Ordered into active Federal service 3 December 2003 at Eleanor; released from active Federal service 28 February 2005 and reverted to state control
