Redesignated 28 June 1880 as Company A, 15th Infantry

Redesignated 25 June 1881 as Company G, 16th Infantry

Redesignated 11 May 1887 as Company G, 5th Infantry

Mustered into Federal service 11 May 1898 at Camp Bushnell, Ohio, as Company G, 5th Ohio Volunteer Infantry Regiment; mustered out of Federal service 5 November 1898 at Cleveland

Reorganized 14 April 1899 as Company G, Unattached Infantry

Redesignated 14 July 1899 as Company G, 5th Infantry

Mustered into Federal service 19 June 1916 at Camp Willis, Ohio; mustered out of Federal service 15 March 1917 at Fort Wayne, Michigan

Called into Federal service 15 July 1917; drafted into Federal service 5 August 1917

Reorganized and redesignated 25 September 1917 as Company G, 145th Infantry, an element of the 37th Division

Demobilized 10 April 1919 at Camp Sherman, Ohio

Reorganized and redesignated 23 February 1921 in the Ohio National Guard at Norwalk as the 4th Separate Machine Gun Company

Reorganized and redesignated 1 July 1921 as Company G, 145th Infantry, and assigned to the 37th Division (later redesignated as the 37th Infantry Division)

Inducted into Federal service 15 October 1940 at Norwalk

Inactivated 13 December 1945 at Camp Anza, California

Reorganized and Federally recognized 26 March 1947 in the Ohio National Guard at Norwalk as Company G, 145th Infantry

Ordered into active Federal service 15 January 1952 at Norwalk

(Company G, 145th Infantry [NGUS] organized and Federally recognized 25 January 1954)

Released from active Federal service 15 June 1954 and reverted to state control; Federal recognition concurrently  withdrawn from Company G, 145th Infantry (NGUS)

Reorganized and redesignated 1 September 1959 as Company A, 1st Battle Group, 145th Infantry

Reorganized and redesignated 1 April 1963 as Company A, 1st Battalion, 145th Infantry

Converted, reorganized, and redesignated 15 February 1968 as Company C, 612th Engineer Battalion, and relieved from assignment to the 37th Infantry Division

Ordered into active Federal service 10 November 2004 at Norwalk; released from active Federal service 5 August 2006 and reverted to state control

Reorganized and redesignated 1 September 2007 as the 945th Engineer Company
