  • Constituted 28 December 1917 in the National Army as Evacuation Hospital No. 12
  • Organized 3 January 1918 at Fort Riley, Kansas
  • Demobilized 7 July 1919 at Camp Dodge, Iowa
  • Reconstituted 10 November 1936 in the Organized Reserves; concurrently consolidated with the 12th Evacuation Hospital (constituted in July 1923 in the Organized Reserves as Evacuation Hospital No. 12; organized by 13 April 1925 at Boston, Massachusetts, as the 12th Evacuation Hospital) and consolidated unit designated as the 12th Evacuation Hospital
  • Ordered into active military service 25 August 1942 and reorganized at Fort H.G. Wright, New York
  • Inactivated 6 January 1946 at Camp Kilmer, New Jersey
  • (Organized Reserves redesignated 25 March 1948 as the Organized Reserve Corps)
  • Redesignated 4 January 1952 as the 12th Evacuation Hospital, Semimobile; concurrently withdrawn from the Organized Reserve Corps and allotted to the Regular Army
  • Activated 1 February 1952 at Fort Lewis, Washington
  • Reorganized and redesignated 10 June 1953 as the 12th Evacuation Hospital
  • Inactivated 15 December 1970 at Fort Lewis, Washington
  • Activated 16 January 1983 in Germany
  • Reorganized and redesignated 16 January 1992 as the 212th Surgical Hospital
  • Reorganized and redesignated 16 October 2006 as the 212th Combat Support Hospital
