  • Reorganized and redesignated 1 March 1963 as Headquarters and Headquarters Detachment, 109th Medical Battalion, and relieved from assignment to the 34th lnfantry Division
  • Reorganized and redesignated 1 October 1995 as Headquarters, Headquarters and Support Company, 109th Medical Battalion (organic elements concurrently organized from new and existing units in the Colorado, Iowa, and South Dakota Army National Guard)
  • Ordered into active Federal service 24 january 2003 at home stations; released from active Federal service 23 January 2004 and reverted to state control
  • Ordered into active Federal service 2 April 2008 at home stations; released from active Federal service 9 March 2009 and reverted to state control
  • Reorganized and redesignated 1 September 2007 as Headquarters and Headquarters Detachment, 109th Medical Battalion (former Companies A, B, and C as the 209th Medical Company, the 730th Medical Company, and the 928th Medical Company - hereafter separate lineages)
  • Location changed 1 September 2009 to Coralville
